MARYLAND PROGRAM

MD MVA Motorcycle Safety Instructor Application – This link will be used to apply to work as a MD Motorcycle Safety Instructor. Select the “Driver Services” link. On the next page click “Apply for Motorcycle Safety Instructor Certificate” under the Professional Licenses Section. During this application you will need to attach a Driver Record, MD MVA Criminal Record Request and the MD MVA Physical Examination forms (see below). Finally, once you have completed the application, you should print and save the application confirmation number and it should be formatted something like this (0-XXX-XXX-XXX). In a rare instance where MVA loses the application, this will help them track the status.

Out of State Driving Record Request (if applicable) – If you hold a Maryland driver’s license, the MVA has indicated they will retrieve your driving record directly. If you have an out-of-state license, you must obtain a copy of your driving record from that state, showing at least three years of history. This record must be included with your application.

Criminal History – If you are a Maryland resident, complete the attached form Maryland Criminal History Request Form CS-011. If you are an out of state resident, you need to conduct a background check in your state and mail it in to their office at the following:

Physical Condition and Driving Instructor History Certification Form – This link will be used to download the Physical Condition and Driving Instructor History Certification which is required to self certify that you are in good physical condition and capable of performing the duties required of a certified driving instructor. Additionally, the letter must confirm that you have not had a driving instructor certification suspended or revoked in Maryland or any other state while serving as a driver instructor, regardless of the type of instruction. This certification must be included with your application.

Certified Transcript of Education – Please submit a certified transcript or a copy of your diploma from the most recent educational institution you attended. This transcript must be included with your application.

Motorcycle Safety Foundation Certification – Please submit the Motorcycle Safety Foundation Certification showing course completion. This certification must be included with your application.

NEW JERSEY PROGRAM

NJ MVC Motorcycle Safety Instructor Application – This link will be used to apply as a VA Motorcycle Safety Instructor. NJ instructors are required to complete an application for both initial certification and annual renewals.

NJ Fingerprint Request – This link will be used to request and undergo a fingerprint examination from the NJ Motor Vehicle Commission.

PENNSYLVANIA PROGRAM

Criminal History (Required Annually) – This link will be used to request an individual criminal history check from Pennsylvania State Police. If you are APPROVED as a PA Instructor, this fee will be reimbursed directly through the ADP Payroll System by using the “Expense Voucher” Link below. However, you must submit documentation on the Expense Voucher proving that you made a payment (invoice, sales receipt, etc.). PLEASE DO NOT EMAIL THIS DOCUMENT BUT INSTEAD ATTACH THIS USING THE “EMPLOYMENT ATTACHMENTS” LINK BELOW.

PA Driver Record (Required Annually) – This link will be used to request a “FULL HISTORY” individual driving record from Pennsylvania Department of Transportation. If you are APPROVED as a PA Instructor, this fee will be reimbursed directly through the ADP Payroll System by using the “Expense Voucher” Link below. However, you must submit documentation on the Expense Voucher proving that you made a payment (invoice, sales receipt, etc.). PLEASE DO NOT EMAIL THIS DOCUMENT BUT INSTEAD ATTACH THIS USING THE “EMPLOYMENT ATTACHMENTS” LINK BELOW.

CPR and First Aid Certification (Required Every 2 Years) – This link will be used to access an online CPR and First Aid Course. PLEASE DO NOT EMAIL THIS DOCUMENT BUT INSTEAD ATTACH THIS USING THE “EMPLOYMENT ATTACHMENTS” LINK BELOW.

Child Abuse Certification (Required Every 5 Years) – This link will be used to request an individual child abuse history check from Pennsylvania Department of Human Services
From here you will need to create an Individual Account, complete the required questions and submit the application. Click This Link for detailed instructors on this process. Once you have answered all the questions and completed the application, you will receive a confirmation that the application has been submitted. Typically, the results are available within a few hours. However, it can take up to 14 days to get the final certification. You should receive an email informing you that your clearance has been completed. After you receive this email, you will be able to log back into the system to view and print the certificate for your records which is good for 5 years. If you are APPROVED as a PA Instructor, this fee will be reimbursed directly through the ADP Payroll System by using the “Expense Voucher” Link below. However, you must submit documentation on the Expense Voucher proving that you made a payment (invoice, sales receipt, etc.). Resume (Required for Initial Certification) – This is a required document. PLEASE DO NOT EMAIL THIS DOCUMENT BUT INSTEAD ATTACH THIS USING THE “EMPLOYMENT ATTACHMENTS” LINK BELOW.

PennDOT Forms – This link will be used to complete the required PennDOT instructor forms.

VIRGINIA PROGRAM

VA MVC Motorcycle Safety Instructor Application – This link will be used to apply to work as a VA Motorcycle Safety Instructor. Instructors are required to complete an application for the initial certification. In addition, instructors must submit a 5 year Driver Record.

VA DMV Driving Record – This link will be used to request an individual driver record from the VA Department of Motor Vehicles. Select the “Request Online” link.

Employment Attachments – This link will be used to attach the Driver Record.

OTHER INFORMATION

Instructor Shift and Location Survey – This link will be used by an instructor to complete a short survey to identify the preferred teaching location(s) and availability.

MakeShift – This link will be used to schedule and track time. Instructors will use this link to check in / check out of a location.

ADP – This link will be used to make changes to your personal information, direct deposit and tax withholdings. Instructors can also use this link to check recent pay checks.

MSA Apparel Website – This link will be used to order MSA Apparel. Instructors can order apparel directly from this website and will be reimbursed up to $40 for every 50 hours of teaching. The maximum reimbursement for the year will be $120 maximum. Please remember to save your receipt(s) and submit these receipts using the MSA Expense Voucher link.

Travel Voucher – This link will be used to request a travel reimbursement. To request travel reimbursement, ALL the following conditions must be met:
• You must have authorization from the training center’s State Coordinator to be on Travel Status prior to incurring any travel related expense.
• Travel expenses are reimbursable from home to the alternate work site based on the shortest distance and time per Google Maps.
• You must submit documentation on the Travel Voucher proving that you made a payment (invoice, sales receipt, etc.).
• Travel to an alternate work site must be more than 50 miles from traveler’s residence and Permanent Duty Station (PDS).
• Travel over 100 miles requires Lodging.

Daily Lodging Reimbursement Rates are based on the government standard. These rates can be found at Daily Lodging Rates

Mileage Reimbursement is calculated using the following formula:
Step 1: Distance from Home to Permanent Duty Station
Step 2: Minimum Mileage = (50 miles each way or 100 miles round trip)
Step 3: Commute Mileage = (Greater Number in Step 1 or Step 2)
Step 4: Total Mileage Driven
Step 5: Reimbursable Mileage = Total Mileage Driven – Commute Mileage
Step 6: Reimbursement Amount = Reimbursement Mileage x Government Rate

Daily Per Diem is not authorized or paid.

Expense Voucher – This link will be used to request a expense reimbursement. You must submit documentation on the Expense Voucher proving that you made a payment (invoice, sales receipt, etc.).

Supply Request – This link will be used to request supplies needed for a specific location.

Motorcycle Repair Request – This link will be used to request motorcycle repairs needed for a specific location.

Property Transfer – This link will be used to document a property transfer.
