EMPLOYMENT FORMS

When selecting the employment forms, you will be required to electronically sign these documents by using the Adobe Sign application.

Direct Deposit Authorization (New and Current Employee) This form is for all employees that want to designate a direct deposit account for payroll.

General Employment (New Employee) These forms are for all employees and include the following:
· Federal I-9 Form and Related Instructions for Employment Eligibility Verification (attach the required verification documents);
· Federal W-4 Employee's Withholding Certificate; and
· Federal W-11 Hiring Incentives to Restore Employment (HIRE) Act Employee Affidavit.

Maryland MW-507 Employee's Maryland Withholding Exemption Certificate (New Employee) This form is specific to any employee that resides in Maryland.

Pennsylvania Department of Transportation (PennDOT) (New Employee) These forms are specific to Motorcycle Safety Instructors that work in Pennsylvania.  The PennDOT Forms include the following:
· Child Abuse History Certification;
· Confidentiality Policy;
· Customer Service Policy;
· Request for Criminal Background Check; and
· Request for PA Driving Record.

Probationary Motorcycle Safety Instructor

Each candidate must meet the following minimum qualifications and expectations for the position:

  • Complete and pass a 2 to 6-week (depends on scheduling) Motorcycle Safety Foundation (MSF) Instructor Preparation Course;

  • Complete and pass a First Aid and CPR Certification Course;

  • Learn and understand the state’s specific requirements / laws for motorcycle licensing and riding;

  • Complete a probationary period and pass an MSF quality control evaluation;

  • Attend professional training to update and refine skills relating to motorcycle safety;

  • Exceptional written and verbal communication skills;

  • Outstanding time management and organizational skills with a high attention to detail;
